Transcript Request FAQ
1. Q: Can I request transcripts over the phone or via e-mail?
A: No, we need your written permission to release information about you.
2. Q: How do I go about getting my transcripts sent?

3. Q: Can I include my narrative evaluations with my transcript?
A: Yes. You can have all, none, or selected narratives included. We do not send narratives unless you request them.
4. Q: Can I have the transcript sent directly to me?
A: Yes. Usually, official transcripts are sent directly to another school and if we send a transcript to you, it is an unofficial student copy. If you need an official copy of your transcript, please mention specifically that you need an OFFICIAL transcript. It will be sent in a sealed inner envelope.
5. Q: May I fax the request to you?
A: Yes. If you want to fax your request, prepare a letter with all the information mentioned earlier and fax it to 937-769-1089. Faxed requests must contain your signature. If you request by fax, please pay any fees by credit card. If you don't, we will send you a bill, which you may pay by check or money order.
6. Q: Can I have you fax the request somewhere?
A: No, I'm sorry we don't fax transcripts. If you need them right away, we can send them via "overnight" delivery, after we receive your request. There is a $25 fee for overnight delivery in addition to any required transcript fees. We can only "overnight" to a street address. We do not deliver to post office box numbers.
